There seem to be multiple such worlds; if a writer doesn't want to deal with the accumulated changes of previous fictional visits, they can go to a variant world that doesn't have any of that. (For example, [[Purple Crayon]] and [[Anime Freak]] were both once RL-1 versions of [[Tom Russell]].) Some of these worlds have been occasionally referred to as "Real Life Minus Two", etc., but any terminology along that line is doomed to get hopelessly tangled.
